The notrious "right hook" is my greatest fear while riding on bike
lanes or side paths. Seeing behind is a serious issue for reclined
    riders.

Due to the damage I struggle to see much behind me beyond its big and red
must be a bus sort of thing! Though London population and generally slower speeds plus some of the bike infrastructure either bypass the junctions or
use traffic lights to control junctions, plus no turn on red and so on.

In general stuff is less car centric in design, which makes sense
considering the mode shares and what not.

No report had been filed because it is so common and the police normally do nothing at all unless there are4 serious injuries.

He didn't have to "see behind". The rider was directly next to him and he was making an illegaol right turn since there was a sign stating "no right turn on red". On a Catrike he might have had at least some excuse but the victim was on a normal bike,

not only did the driver commit a 'right hook' which is itself a traffic violation, but he made the move in an attempt to circumvent the
stoplight. The vehicles in front of him were stopped waiting for the red
right arrow to turn green, he drove around them. He's very clearly one
of those people to whom the law doesn't apply.

There was a pretty clear shot at his licence plate and there should
have been some damage to his right side. Should've been easy to track
    him down.

Looking at the junction in street view looks very car centric, design and parked cars particularly considering how central it is. I believe US
doesnrCOt go for speed or traffic cameras as much as other countries, people will drive at speed that the road feels safe, so big wide roads like that
    will encourage higher speeds.

Average speed cameras I find has a calming effect ie stops pulsing from
drivers speeding then braking for the camera and then accelerating after.

As does the 20mph limit which at least for urban or suburban areas makes negligible difference in car journeys times but means that cars are more
human speeds and reduce traffic fatalities and encourages active travel and
    all that.

In theory yes though not stopping is definitely a thing, and some
organisations have prescribed ways of getting video footage so may not
    accept and all of that.
