    I finally was able to get my LS-DOS 6.2.0 Level AH disk for Model II/12 imaged, with many thanks to Pete Cetinski, who has put the image into his Model2Archive on github ( https://github.com/pski/model2archive/tree/master/Software/Other/LS-DOS ). I don't recall who gave me this disk, but it was successfully imaged and is available for download in ImageDisk format, thanks to Pete.
