Ok I am not even going to say what happened, because you could see it
    coming a mile a way

The question I have is about Rocco.
So, why did Lulu call Isaiah for clandestine medical help?

When did they become pals? Did I miss a scene where he was somehow clued
    into RoccorCOs gun work?

Why did she have to call anyone for clandestine medical help? Rocco was already treated at the hospital for the injury.
How is the cover story of Rocco cutting himself by accident inconsistent
    with an infection?

It's not. Maybe the infection came from an insufficiently cleansed wound
and Lulu worried there could be gunshot residue in it. Or maybe Lulu
worried that closer inspection of the wound would reveal that it
couldn't possibly have been made with a kitchen implement. Or maybe this
was just a lame plot ploy to draw Isaiah into the conspiracy. I won't
have a chance to watch this week's episodes until the weekend, so I
don't know what the circumstances are and probably shouldn't be
    commenting on it.

Before I saw the episode, I had also started to wonder if Lulu worried
that the ER staff might ask how Rocco managed to cut his dominant
hand--the hand that would have been holding the kitchen knife. That
speculation was just as wrong as the other I made above.

Lulu should have just taken the kid to the ER--her reaction was
irrational. She never mentioned to Fake Nathan that she thought Liz had suspected something was fishy about the wound when she treated it. What
was so fishy about it? And did she worry that the nurse or doctor who
would treat Rocco's infection at the ER might be able to tell that the
cut had been caused by firing a gun? How do you cut your hand by firing
    a gun?

You cut your hand by not having it clear of the slide on a
semi-automatic firearm. The slide is the movable top portion which is
forced backward with each shot on most semis. A spring usually pushes it
back forward for the next shot.
It's a real-life risk of injury, especially for an inexperienced shooter
    like Rocco.

Wow. Thanks for the explanation. I'm not a gun person, so really had no
idea. I guess there's a good chance an ER crew would recognize such an
injury then, which would explain why Lulu didn't want to take Rocco to
    the hospital.
