Seems similar to the Croydon tram crash:

A tram derailed in central Milan on Friday afternoon, hurtling off its
tracks at high speed and slamming into a building, killing two and leaving about 50 people injured.

DoesnrCOt look like the fatalities were caused by being thrown through the windows and then being trapped by the vehicle ending up on its side though,
so more like a regular RTA except the vehicle concerned is normally on
rails. Considering the environment they operate in IrCOm surprised we donrCOt hear about more incidents, perhaps they donrCOt reported . Not that we have many systems in the UK
but Nottingham had that incident the other week where a Tram was derailed
in a collision with a bus.
Quite a knock when you see the photos but unlike Milan no building for the
tram to collide with after leaving the track. <https://www.bbc.co.uk/news/articles/cdxgvzp1523o>

I recall on my last visit to Blackpool which was before the rebuild one of curves at Fleetwood showed evidence of several derailments in the road
    surface.

Similar in the sense that a modern tram over-speeded on a tight curve, derailed, and flew off the track causing fatal casualties, because of
driver inattention. It comes as something of a surprise that even modern
trams seemingly have no over-speed protection l

Collisions on shared road sections are quite common, but over-speeding on curves is fortunately much less so.
